Liftmaster Multi-Function Door Control Console 4 Terminal Wall Button Old Style
This is the REALLY old style wall control for Chamberlain operators with the dip switch receiver settings. They are 4 terminal style with Black, Yellow, White and Red connection points.
I'm unsure what the actual part number for these units is as they pre-date the internet but I did find some numbers on the few units I have that are consistent so I used those for reference.

This unit has a big button to run the door and a lock and light switch.
